MUNICIPAL ASSESSOR’S OFFICE`

I – SECURING CERTIFIED TRUE/PHOTOCOPY OF TAX DECLARATION AND CERTIFICATION OF VARIOUS PROPERTY HOLDINGS OR NON-IMPROVEMENT

This service refers to the issuance of certified true copy of certifications, tax declaration of real property/ies, certifications of various property holdings or non-improvement to be requested from the Municipal Assessor’s Office.

FEES:

Per Certified True/Photo Copy of Tax Declaration/Certification – P 50.00

HOW TO AVAIL OF THE SERVICE:
StepNo.
1. Client fill-up the request form.
2. MASSO personnel Verify records and advise client to pay required fee at Mun.Treasury Office
3. Client pays required fees at Mun. treasury Office.
4. Submit the Official Receipt at the Office.

5. MASSO personnel will prepare the requested documents.
6. Review and signs the document by Fe A. PañaOIC-Mun. Assessor.
7. OIC Fe A. Pana Receives the prepared document/Record, signs after review and release the requested document after client signed the received document.

II – REQUEST FOR TRANSFER OF TAX DECLARATION TO NEW OWNER

This service refers to the appraisal and assessment of real property for transfer of tax declaration to new owner.

REQUIREMENTS:

1. Land Title (2 Certified Photocopies)
2.Registered Deed of conveyance(2 Certified Photocopies)
3.Real Property Tax Payment (2 copies)
4.Transfer Tax Receipt
5.CAR (Certificate Authorizing Registration) 2 Photocopies)
6.Tax Declaration
7.DAR clearance (for Agricultural land) Original Copy
8.Certification of tax payment from 1945 to present (MTO)
9.Approved survey plan

HOW TO AVAIL OF THE SERVICE:
StepNo.
1. Taxpayer request , fill up the request form for transfer of real property and submit all the requirements and documents
2. Validate the documents, submitted and verification/verifyPrepare field sheet  by Assessment Clerk/MASSO Personnel.
3. Received/validate the documents submitted and verify for 15 minutes by the Assessment Clerk

4. Prepare field sheet, assign ARP and records to the log book by MASSO Personnel
5. Tax map, assign PIN and records conversion by the Assessment Clerk

6. Review and signs the document by Fe A. Paña OIC-Mun. Assessor

7. Typing of documents, FAAS or Tax Declaration /TD by the Assessment Clerk/MASSO Personnel
8. Numbering of Tax Declaration by Assessment Clerk

9. Draw the sketch plan on Tax Declaration  – Assessment Clerk
10. Review and sign the processed TD and endorsement to PASSO by  Fe A. Paña, OIC-Mun. Assessor
11. Submission of the processed TD for approval to the PASSO Every Tuesday & Thursday or as the case may be Fe A. Paña, OIC-Mun. Assessor.
12. Release of approved FAAS to client 1 day after release from  PASSO.

III – REQUEST FOR ASSESSMENT OF NEWLY DECLARED REAL PROPERTY

Newly declared properties such as new building, machinery or land need to be assessed before it can be issued a tax declaration, TD will been serve as the permanent record and basis of real property tax.

REQUIREMENTS:

1. Building Permit
2. Blueprint of the approved building plan/survey plan
3. Photocopy of occupancy permit
4. Sworn statement of cost of machinery (new machinery) or receipt
5. DENR certification of list of claimants (newly discovery or newly declared land)
6. Deed of conveyance duly registered from ROD (newly declared land)

HOW TO AVAIL OF THE SERVICE:
StepNo.
1. Request and fill up request form and Submit all needed document to Municipal Assessor’s office
2. Receive request form and advise client for the conduct of ocular inspection 1hour after approval of the Mayor.
3. Receive and evaluate documents for declaring new property 2-3 hours / day by MASSO personnel
4. Conduct ocular inspection after 2 hours by Fe A. Paña -OIC Mun. Assessor.

5. Prepare inspection report / field sheet /TD or FAAS by Fe A. Paña, OIC-Mun. Assessor.

6. Tax map and assign PIN on TD by Fe A. Paña, OIC-Mun. Assessor.
7. Process the TD for approval of the PASSO.
8. Review, signing and endorsement of processed FAAS,  by Fe A. Paña, OIC-Mun. Assessor.

IV – REQUEST FOR ANNOTATION / CANCELLATION OF MORTGAGE, ENCUMBRANCES, BAILBOND, ETC. ON REAL PROPERTIES (LAND, BUILDING OR MACHINERY) IN THE ASSESSMENT RECORD

This service refers to the real property subject for annotation / cancellation of mortgage, encumbrances, bail bond, etc. in the assessment record.

REQUIREMENTS:

1. Reg. notice / release of mortgage

2.Reg. notice / release of bail bond

HOW TO AVAIL OF THE SERVICE:

1. Taxpayer request and fill up the request form for annotation of mortgages etc.
2. Submit all the requirements and documents
3  OIC Mun. Assessor Fe A. Paña evaluate the submitted documents.
4. Annotate tax declaration, by  Fe A. Paña, OIC-Mun. Assessor.
